Fiscal Sponsor Application- Pre-approved Grants - Model Type C

Philanthropy In Action (PIA) provides fiscal sponsorship to community organizations and groups with a complimentary mission. The mission of PIA is to catalyze vision and support for change initiatives and collaborative causes that strengthen and foster better communities. We empower donors through value-aligned giving and are a home for nonprofit fiscal sponsorships (Type A) and start-up nonprofit organizations (Type C – pre-approved grants).

Please answer the following questions. Your application will be reviewed, which may result in the need for an interview. PIA will contact you within thirty days to discuss the next steps.

1. Provide the mission and vision of the nonprofit organization:

3. Has the grant been submitted to a private foundation? Provide the name of the private foundation and grant request amount.

4. Provide a synopsis of the grant request. Include purpose, goals, activities, geographic scope, and target population. The synopsis should be short and succinct.

5. Provide a list of the board of directors.

6. Provide the following:

a.) Most recent profit and loss statement

b.) Statement of assets

c.) Current operating budget

7. Do you propose any political or lobbying activity, and if so, what?
